I don't think that's a good way to remove attachments...

Reason being, the attachment display name, what is returned by @attachments may not be the name the attachment has internally.

For a example, if a user attaches a file called test.doc then deletes it from the document before saving it, and attaches it again, the internal name to notes won't be test.doc
